  • Highest level of education
  • Legal marital status with spouse's name (maiden)
  • Usual occupation
  • Veteran information, if applicable, including the DD-214
  • Your name, address, and relationship to the deceased
  • Life insurance policy or a pre-arranged funeral policy, if applicable
